Community Health Worker Certificate

Students who complete this certificate program are eligible to be certified by the Ohio Board of Nursing as Community Health Worker.  The Community Health Worker will work in the community as an outreach resource assisting clients in six major areas: health care, community resources, communication skills, individual & community advocacy, health education and skills and responsibilities.  Practicum experiences in the community will be a major component of the certificate.  A minimum grade of C- is required in all courses and an overall certificate grade point average of 2.00.  An acceptable Bureau of Criminal Identification and Investigation (BCI&I) report is required.

 

Community Health Worker Certificate Classes:
  • CHWR2710 - Community Health Worker I
  • CHWR2730 - Health Care Across the Lifespan
  • CHWR2750 - Community Health Worker Directed Practice/Seminar

 

This is a FLEX program of study and DOES NOT follow the College's calendar. The classroom meeting days and time will vary. The student must read the printed comments for actual meeting dates, classroom times, and practical hours. This program is offered approximately twice per calendar year, usually in the Fall and Spring quarters.

Upon successful completion of a certificate program, a Certificate Request form must be completed in the Student Records Office in order to receive a certificate.
