1. My need to take a distance course is:
  High - My career and/or life circumstances prevents me from attending traditional classes.
  Moderate - I could take this class on campus or substitute it for another class.
  Low - I think it will be easier than traditional classes.
     
2. How often will you be able to devote time to your distance course?
  Everyday.
  As often as required.
  Once or twice a week.
     
3. How much time will you spend studying and interacting with your distance course?
  As much time as it takes to complete the course.
  The same amount of time as a traditional campus class.
  Less time than a traditional campus class, because it will be easier.
     
4. Are you motivated to meet deadlines?
  I always get my work done on time.
  I am a procrastinator and I need to be reminded and guided to meet my deadlines.
  I always turn in my work late.
     
5. How do you rate traditional classroom discussion with an instructor and other students?
  I can communicate effectively through email, discussion boards, and chat rooms.
  It is an important and essential aspect of my educational experience.
  I am shy and do not communicate effectively with my instructor and other students.
     
6. When the instructor hands out directions for an assignment...
  I can read and understand the directions.
  I need to hear the directions in order to understand my responsibilities.
  I need help to understand the directions.
     
7. I understand information when it is presented:
  in a visual and auditory format. (text, pictures, diagrams, video, lecture, tapes, sound)
  in a lecture format.
  I will need help with understanding information in a distance course.
     
8. Are you familiar with the accessibility options available for your computer?
  Yes. I have changed the accessibility options or I own adaptive software and hardware that would allow me to succeed with an online course.
  No. I need help understanding how to change the accessibility options for my computer to take a distance course.
  Does not apply. I do not use the accessibility options on my computer.
     
9. Where would you go to get help if you had a problem?
  The Instructor.
  NC State College's Online support.
  Another student in the class or a friend.
     
10. What is the current speed of your modem?
  Cable or DSL

  I don't know what a modem is.
     
11. What will you do if your computer breaks down, or your Internet connection fails?
  I will use a computer at a friend's house, the open lab on campus, or the library.
  I will try to fix it.
  I will ask the instructor for extensions on all my assignments.
     
12. Are you comfortable completing all course work on a computer?
  I am very computer literate.
  I would like to learn more about computers.
  I don't like computers, but will use one if I have to.
     
13. Are you comfortable using a mouse?
  Yes.
  No. I always click the wrong thing.

14. My keyboarding skills are:
  Very strong. I type over 20 words per minute.
  Okay. I am getting better. I type less than 20 words per minute
  I can't type.
     
15. Are you comfortable using an Internet browser?
  Yes, I browse the web all the time.
  No. I would like help.

16. Are you comfortable using email?
  Yes, I have my own email account and know how to use my NCSC account.
  I do not have an email account. I would like to learn about email.
  I don't like email.
     
17. Are you comfortable with file management: saving, deleting, and moving files on your computer?
  Yes. I am computer literate.
  I would like to learn about file management.
  No. I can never find my files after I save them.
     
18. Do you feel comfortable downloading and installing software programs?
  I feel comfortable. I have done this many times.
  I will ask for help.
  I will not and/or cannot download and install any software on my computer.
     
19. Do you know how to use standard word processing software such as Microsoft Word?
  Yes, I use word processing software all the time.
  I have used a word processor a few times. I would like help.
  No. I have never used a word processor.
     
20. What kind of delivery methods would you prefer to use for a distance course?
  Online - delivered 100% via computer.
  Video - may use VHS tapes or streaming video online, exclusively.
  Audio - may use cassette tapes, CDs, or streaming audio online, exclusively.
  None - I don't believe distance learning is for me.
